Players With Team - How To Register

This instruction is for players who have a team to play on. If you are a player (or parent of a player) and are looking for a team, go to How To Find A Team. If you are on a Jewish Community Center (JCC) Team, do not follow these instructions, contact the JCC.

Step 1 - Get Your Tin From Your Coach

Contact your coach to determine whether you or your coach will register on-line. If you are registering on-line, you will need the Fall 2007 Team Identification Number (TIN) which you must get from your coach. This is not the same number as last year. The correct number is essential to making sure that your child ends up on the right team.

Step 2 - Register The Player On-Line

Click on the Players With Team link and register your player on-line. You will need the child’s name, birthday and an e-mail address.

Before you submit it we recommend that you print a copy of what you send for your records. If a player does not have access to the Internet, anyone associated with the team can complete this form for the player.

Step 3 - Complete the CYSA Form

Your coach will provide you with a CYSA Registration Form. Please make sure that the child's name matches the birth certificate/passport exactly (no nicknames). The form needs to be signed as follows:

1. By the Parent in two (2) places
2. The player does not need to sign

Step 4 - Sign the Spectator Code of Conduct form

Your coach should provide you with this form. It is also available to download and print by clicking here for the English version or aqui for the Spanish version.

Step 5 - Give CYSA Form, Fees, Picture and Birth Certificate and Spectator Code of Conduct form To The Coach or Team Manager

Let the Coach/Manager know that you completed on-line registration and provide your coach with each of the following:

1. PICTURE - a 1”x 1 1/2” picture of the face of the player. Write thier name, team name, & TIN# on the back of the picture. (School or passport quality photo)
Requirements:
Picture size must be approx 1” by 1.5”. Small photos or large photos must be rejected.
Picture must be of head and shoulders only.
Player must be looking at the camera
Player can't be wearing a hat or glasses.

2. Check for $70 ($110 if your team is registering after May 24, 2007) payable to the coach if he/she is submitting one check for entire team or to SF Vikings League. Returned check fee of $25.00 applies.

3. The completed CYSA 1601 form. Must be signed in 2 places.

4. A copy of your child’s birth certificate or passport ( these will not be returned)

Return the above items (clipped together in the order above) to the team coach or manager before their registration appointment. Do NOT bring them to the Vikings office because the coach or team manager is to submit the entire team's paperwork at the same time. However, if your child is adding to the team "late" (after the team has completed registration) then paperwork can be dropped off at the Vikings office but must have the team name and TIN included.
