Teams must be aware of the restrictions on poaching players from other teams. Once a player has signed a USYSA form and submitted it with fees to our league, they may not be solicited to join another team for the new season. Appointments for travel teams start on April 17h so as of that date, it is possible for a player to be registered for the 2007/08 year. It is your responsibility to confirm that the players are not already on a registered team. Step
2: Register
Your Team On Line With The Viking League 1. Fall 2007 Team Identification Number (TIN) via email within 2 business days. This will be different than your TIN from last year. Use the TIN on every correspondence to the Vikings Office; it is essential to identifying your team and keeping everything straight. If you lose it, go to the web and click on FIND MY TIN to find it. (Please keep in mind that not all fields are made available for public view). 2. A package containing CYSA Membership forms and detailed registration instructions via mail within one week. These forms require original signatures. Step 3: Email to Schedule An Appointment Email the office to schedule
an appointment to register your team. We will start accepting
email correspondence for appointments April 12th for select/travel teams. Select teams registration can begin April 17th, 8:30 am - noon in half hour increments. Last appointment day to register select/travel teams is June 5th to have paperwork ready for September play. All Registration steps (online and paperwork) must be 100% completed by your appointment or it will be cancelled! These forms may only be delivered to the office during a scheduled appointment. DO NOT DROP THEM OFF. 1. CYSA YOUTH SOCCER MEMBERSHIP FORMS for each player signed by a parent in two places. Players do not need to sign the forms unless they are eighteen years or older. 2. CYSA 1628 Adult Registration Form and Team Management Disclosure Form: *Rev. 11/2006*. You must provide photocopy of a coach license for all coaches and assistant coaches, team managers and team assistants. (please discard all OLD forms as they cannot be accepted.) 3.
BIRTH CERTIFICATES/Current passport Must be a clear copy
of the entire birth certificate (copies only as they will not be
returned) from every player (all players must submit birth certificates
even if they played before) We are required to keep records on file
for three years so we cannot pull the birth certificates from the
prior year.
5. CHECK(S) Payable to the SF Vikings Soccer CLUB in the amount of $27 per player if your team will complete their registration by June 5th. You may submit one or multiple checks, although we prefer one check. Write team name, TIN, and player name in memo field. Please check with your club (sometimes your club will cut a check for the team) Step 5: Attend Appointment and Obtain Field Request Form All Registration steps (online and paperwork) must be 100% completed by your appointment or it will be cancelled! Paperwork should be papercliped with photo on top followed by check (if applicable), USYSA membership form, proof of brith (any translation document) PUT THE YOUTH FORMS IN LAST NAME ALPHABETICAL ORDER. If your paperwork has been filled out properly, you will receive a stamped Park and Rec Practice Field Request Form. Be ready to fill in your practice field request form and submit to Park and Rec at the Old Log Cabin in Golden Gate Park. They will time stamp the forms to set priority. Deadline to submit is July 6th. Step 6: Complete District Paperwork In
addition to registering with the Vikings League you may have other paperwork requried by District 1. Check with you Club on any additional requirements. Additionally, you must follow any playing league deadlines and requirements. Step 8: District Registrar Review and Stamping of Your Paperwork
All Select Team registration materials must be reviewed and approved by the District Registrar. After the Vikings League processes your paperwork, we will deliver it to the District Registrar. At your registration appointment, please be sure to let the league know the first day you will need your paperwork and passes for a tournament, league play, etc., and please make sure that you allow enough time for processing from the date of your appointment to the date you need the paperwork and passes. Step 9: Submit to the SF Vikings Club any dues, fees, forms In
addition to registering with the Vikings League you must pay your
Annual Vikings Club dues of $25.00 plus any other fees as requried
by your team coach/manager to cover expenses. Please make those
checks payable to SF Vikings Soccer Club. Please put your team name,
age and player last name in the check memo. Parent Pledges should
also be filled out and submitted to the team manager/coach.
